My Yearly Health Check

A guide and record book to a yearly health check for people with Down syndrome.

This resource has been developed to aid the process of a yearly health check. It has been developed into two parts, the first part is information on your current health status, the second part is to be completed when you visit your Doctor or Specialist Nurse. They will look at the first part of the booklet that you have filled in and then fill the rest in with you. This resource was developed by Dina Whatnell a Clinical Nurse Specialist Lead Development Disabilities at Palmerston North Hospital, who also has a family member with Down syndrome.

NB: The library resources are available for ADSA library members to borrow. Unfortunately we do not sell direct to the general public.
